Our Partnership is made up of the City of Cardiff Council, Vale of Glamorgan Council, Cardiff & Vale University Health Board, Welsh Ambulance Services NHS Trust, Third & Independent sectors and carer representatives. Together, we aim to improve outcomes and well-being for our citizens by:– improving care and support, ensuring people have more say and control;– providing co-ordinated, person centred care and support;– making more effective use of resources, skills and expertise.
